For a peptide to be compounded by a US pharmacy under Section 503A, its bulk ingredient generally has to be part of an FDA-approved drug, have a USP monograph, or appear on FDA’s 503A Bulks List. None of the peptides reviewed in July meets the first two tests, so the Bulks List is the only path. FDA’s Pharmacy Compounding Advisory Committee (PCAC), an outside panel of pharmacists, physicians and scientists, advises the agency on which substances belong there.

How the votes went

Peptide Committee recommendation Vote
BPC-157 Add to 503A Bulks List 8–6, 1 abstention
KPV Add 8–6, 1 abstention
TB-500 Add 8–6, 1 abstention
MOTS-c Add 7–5, 2 abstentions
Epitalon Add Narrow majority
Semax Add Narrow majority
Emideltide (DSIP) Do not add 7–6, 1 abstention against

Vote counts as reported by AJMC [2]. The nominations tied each peptide to specific conditions, including ulcerative colitis, wound healing, obesity, osteoporosis and opioid withdrawal [2]; the BPC-157 recommendation was tied to a proposed ulcerative colitis use [3].

Why FDA’s own reviewers disagreed

FDA staff scientists recommended against adding any of the seven peptides [2]. During the meeting, reviewers pointed to a general lack of quality data on safety and effectiveness, noting that most BPC-157 research comes from rodent studies and that there is very little human clinical trial data [4]. Members who voted no raised similar points, along with a concern that a Bulks List entry could suggest a level of FDA evaluation that hasn’t taken place [4].

What happens next

  1. FDA decides whether to accept the committee’s recommendations. It is not bound by them.
  2. If it moves forward, FDA publishes a proposed rule and opens a public comment period.
  3. A final rule would add the peptides to the 503A Bulks List. Only then can pharmacies compound them under that pathway.

“FDA still must formally adopt the advisers’ recommendations before the peptides may be legally compounded,” Scott Brunner of the Alliance for Pharmacy Compounding told AJMC [2]. HHS Secretary Robert F. Kennedy Jr. has publicly expressed support for compounding access to peptides [1].

A second PCAC meeting, planned before the end of February 2027, is set to review five more peptides: GHK-Cu, Melanotan II, LL-37 (cathelicidin), Dihexa acetate and PEG-MGF [5].

What hasn’t changed

  • None of the six peptides is an FDA-approved drug.
  • Products sold as “research use only” are still sold as laboratory chemicals, not as medicines.
  • BPC-157 and TB-500 remain on WADA’s prohibited list for athletes.

For the science behind two of these compounds, see BPC-157 vs TB-500. KPV, BPC-157 and TB-500 are three of the four peptides in the KLOW stack.
